AAHA Player Verification Process

Instructions


The Atlantic District requires that all Players, other than Adult or Disabled Disciplines, be verified by the District Registrar prior to being approved to a USA Hockey Team Roster. Proof of both the players birthdate and citizenship are required to complete the verification process. Once the player is registered with USA Hockey, a Birth Certificate or another form of Government issued ID is required to be submitted for review and verification.


You will need to your current season USA Hockey # to complete. Please provide .pdf files, not .jpeg.
